FOLLOW MY HEALTH
Our Follow My Health Service allows us to help better serve our patients. Through this service patients can request appointments, renew prescriptions, view test results, pay your bill and update your health records. We can also communicate through the secure messaging service.
APPOINTMENTS AND CANCELLATIONS
Please call (231)876-1122 to make an appointment. Email firstname.lastname@example.org for non-emergent appointments. Patients are seen by appointment only, so please call in advance. If you cannot keep your appointment, we would appreciate a phone call to cancel the appointment so that the time slot can be made available to other patients.
PATIENT EDUCATION PROGRAMS
Our trained staff provides diabetes education, smoking cessation programs, heart health screenings, blood pressure clinics, as well as a host of programs especially tailored to your individual needs.

Questions requiring Provider input are forwarded to your Provider.
Nurses are available for calls on weekdays from 7:30 am to 5:30 pm and Saturdays from 8:00 am to Noon.
