Our Family Services include; Physicals, Well-Women Care, Well-Baby Care, Well-Child Care, Sick Visits, Sports Physicals and Group Visits

As your Patient Centered Medical Home, we use teamwork and information technology to improve your care and coordinate with other qualified professionals.

We also provide Occupational Services such as; Workers Comp Services, Pre-Hire Services, and Drug Screening and Alcohol Testing

Cadillac Family Physicians care team will personally facilitate and coordinate the best care for you and your family.

Effective 4/10/2023, Cadillac Family Physicians began optional masking for Providers, Patients, and Staff. When scheduling an appointment, you will still be asked if you have any known upper respiratory symptoms. If so, you will be scheduled with a Provider in our private entrance exam room or offered a virtual visit.
Masks are required in the private entrance exam rooms due to the possibility of contagious symptoms.

As your Patient Centered Medical Home, we use teamwork and information technology to improve your care and coordinate with other qualified professionals, including hospitals, referral providers, nursing homes, pharmacies, and other related community resources that you may need.

New Patients

New patients must complete a new patient application and a New Patient Healthcare Information Release form.
Completion of the application does not guarantee acceptance as a patient with a provider at Cadillac Family Physicians PC.
